As a Volunteer for Sunshine State Labrador Rescue, Inc. you agree to release and hold harmless Sunshine State Labrador Rescue, Inc., its agents, and volunteers from any liability for injuries or damages caused by the negligence of Sunshine State Labrador Rescue, Inc., its agents and volunteers. Furthermore, you agree to release and hold harmless Sunshine State Labrador Rescue, Inc., its agents, and volunteers from any injuries or damages caused by a dog owned by Sunshine State Labrador Rescue, Inc.

I certify that all information I have provided in this application is true, complete, and accurate. I understand and agree to the following:

1. Depending on my volunteer role and level of access, I may be required to pass a background check.
2. All positions are volunteer-based and I will not be paid for my time or services.
3. If I am under 18 years of age, my parent must accompany me while volunteering.
4. Sunshine State Labrador Rescue, Inc. reserves the right to refuse volunteer services for any reason.
